Referring a patient for exercise physiology, Five Dock
For GPs, specialists, support coordinators, plan managers and case managers.
We are two accredited exercise physiologists inside SportsFit Health and Rehab at Five Dock, alongside physiotherapy and sports chiropractic. Referrals can be sent by fax, by email or through your practice software.
One of the two, Kevin Wong, is also a registered physiotherapist, which is useful for patients who need hands-on treatment and a loading program without two separate appointments.
What we need from you
If something is missing we will ring you rather than send the patient back.
Chronic Disease Management
A completed referral form with your provider number, the plan date and the conditions covered. Patients claim the Medicare rebate and pay a gap. We send a written report back after the initial assessment and at the end of the block.
NDIS
No referral is required. What helps is the goals wording from the plan and how the plan is managed. We are not NDIS-registered, so we can see plan-managed and self-managed participants only.
Workers compensation
The nominated treating doctor's referral and the claim number. We send an initial assessment report and progress updates to the insurer and to you.
Aged care
Home Care Package and Support at Home funded home visits across the Inner West. The care partner or coordinator can arrange it directly.
What you get back
An initial assessment letter setting out findings, objective measures and the program. Progress correspondence at the end of a treatment block, or sooner if something changes that you should know about.
If you would rather have a conversation than a letter, ring the clinic and ask for the treating clinician. We would generally prefer that too.
