We make referrals straightforward. Whether you're a GP placing a patient on a CDM Plan, a Pain Specialist managing chronic pain, an OT or specialist working with NDIS participants, a WorkCover case manager, or a Care Coordinator on Home Care Packages โ€” get in touch and we'll handle the process from there.

What we provide back

Initial assessment letter: A written summary of our initial assessment findings and treatment plan, sent to you within one week of the patient's first visit.

Progress reports: For NDIS plan reviews, WorkCover insurer reports, and HCP plan reviews, we provide structured progress documentation as required.

Discharge summaries: When a patient completes their plan or program, we send a final summary documenting outcomes and any ongoing recommendations.

Frequently asked questions.

Do you bulk bill?
No, we don't bulk bill as a general rule. For Medicare CDM-rebated sessions there is a gap fee โ€” we'll explain this upfront when you book. WorkCover, NDIS, and HCP-funded sessions are billed directly to the relevant funder, so there's typically no out-of-pocket expense for those clients.
Do I need a GP referral?
It depends on funding. For self-paying clients โ€” no referral needed, book directly. For Medicare CDM โ€” yes, your GP needs to provide a CDM referral form. For NDIS โ€” no referral required, but your plan needs to include allied health funding. For WorkCover โ€” yes, your nominated treating doctor needs to recommend EP and your insurer needs to approve sessions. For HCP โ€” depends on your plan manager's process.
Can I claim through private health insurance?
Yes โ€” most private health funds with extras cover provide rebates for Exercise Physiology sessions. We process claims via HICAPS at the time of your appointment, so you only pay the gap. The exact rebate depends on your fund and level of cover.
How long are sessions?
Initial consultations are 45 minutes โ€” enough time to fully understand your situation, run a physical assessment, and develop a tailored plan. Follow-up sessions are typically 30 minutes. Longer appointments are available if needed โ€” let us know when you book.
What should I bring to my first session?
Comfortable clothing you can move in, water, and any relevant paperwork โ€” Medicare card, NDIS plan, GP referral, scan results, or specialist letters. If you have specific concerns or symptoms, bring notes โ€” it helps us cover everything in your initial consultation.
Is the clinic accessible?
Yes โ€” our Five Dock clinic is on the ground floor with wheelchair access. There's accessible parking nearby, and the clinic is on major bus routes. If you have specific accessibility needs, let us know when you book and we'll ensure your visit is straightforward.
Where do I park?
Street parking is available on Great North Road and surrounding streets. There's also paid parking at the nearby Five Dock shopping precinct. If you're attending regularly and parking is a concern, give us a call โ€” we'll point you to the best options.
Are you NDIS registered?
No, we are not registered NDIS providers. This means we work with plan-managed and self-managed NDIS participants โ€” but we cannot bill the NDIA directly for agency-managed participants. If you're agency-managed, you may be able to switch to plan-managed at your next plan review.
Do you run group classes?
Yes โ€” we run Medicare-rebated group exercise physiology classes specifically for type 2 diabetes (item 81115). Eligible patients can claim up to 8 group sessions per calendar year on top of the standard 5 individual allied health sessions. Group classes run with 2-12 participants. Ask your GP about a Group Allied Health Service referral, or get in touch for more information.
