We make referrals straightforward. Whether you're a GP placing a patient on a CDM Plan, a Pain Specialist managing chronic pain, an OT or specialist working with NDIS participants, a WorkCover case manager, or a Care Coordinator on Home Care Packages โ get in touch and we'll handle the process from there.

Initial assessment letter: A written summary of our initial assessment findings and treatment plan, sent to you within one week of the patient's first visit.
Progress reports: For NDIS plan reviews, WorkCover insurer reports, and HCP plan reviews, we provide structured progress documentation as required.
Discharge summaries: When a patient completes their plan or program, we send a final summary documenting outcomes and any ongoing recommendations.
