for chronic conditions.
If your GP has placed you on a Chronic Disease Management Plan (also called a Care Plan, GPMP, or TCA), you can claim back part of your Exercise Physiology sessions through Medicare. Cover up to 5 allied health sessions per calendar year.

If you have a chronic medical condition (lasting six months or longer) and complex care needs, your GP can place you on a Chronic Disease Management Plan. This unlocks Medicare rebates for up to 5 allied health sessions per calendar year — including Exercise Physiology, Physiotherapy, Dietetics, and others.
There are three main planning items your GP might use:
GP Management Plan (GPMP — item 721) — your GP develops a written plan documenting your condition, goals, and needs.
Team Care Arrangement (TCA — item 723) — your GP coordinates care between at least two other health professionals (this is what allows EP sessions to be rebated).
Review of GPMP/TCA (item 732) — periodic review of your plan as conditions change.
Conditions commonly seen under CDM Plans.
Your GP determines if your condition qualifies. The conditions below are the ones most commonly referred to us under Medicare CDM — but if your GP thinks Exercise Physiology will help with your chronic condition, we can almost certainly support you.
Type 2 Diabetes group classes.
In addition to individual sessions, Medicare provides separate funding for group exercise physiology classes specifically for type 2 diabetes — covering up to 8 group sessions per calendar year (item 81115), in addition to your standard 5 individual allied health sessions.
Group diabetes classes are an evidence-based, more affordable way to access ongoing exercise physiology support. They run with 2-12 participants and provide structured, supervised exercise programming alongside the social and motivational benefits of training with others in similar situations.
To access these classes, your GP needs to prepare a specific Group Allied Health Service referral form. We complete a one-on-one assessment first (item 81100) and then enrol you into the appropriate group class.
Eligibility: Type 2 diabetes diagnosis, GP-prepared CDM Plan plus a Group Allied Health Service referral, and an initial individual assessment with us.
